Application - High Risk Insurance Person
Standard Premium Rate Increase Request
Individual Report - Standard Health Insurance Fees
Registered Medical Practitioners and Health Care Facilities may complete the Individual Report form to request the addition of specific CPT code(s) to the Standard Health Insurance Fees (SHIF) Schedule. If approved by the Commission, the CPT code(s) will be added to the fee schedule and published on the department’s website.
Please note: The Standard Health Insurance Fee (SHIF) is the list of fees that are utilized by Approved Insurers in the Cayman Islands to pay claims.

The Complaint Intake Form is the official form used by stakeholders to report complaints related to health insurance matters.
Notification of No Health Insurance Coverage
Registered Medical Practitioners and Health Care Facilities must file an annual report with the Commission detailing the CPT codes and fees used for medical billing.
