Application to Renew a Practising Licence

Renewing Your Practising Licence

This process will take at least 30 minutes and should be completed in one sitting.

This form is used to renew an existing practitioner licence and serves as a renewal for a two (2) year licence period.
  • Each practitioner is required to submit their own renewal application.
  • Payments for the renewal may be made either by the practitioner or by an agent.
  • Renewals may occur within twenty-eight days (Principal List) or sixty days (Institutional List and Provisional List) before the expiration in order to avoid a late fee of CI$100.00.
  • Practitioners registered on the Institutional list who are considering changing affiliation/ facility should submit a new application. A renewal is not permitted in this case.

 

If your practice licence has expired over five (5) years or more, you may be required to submit a new application.

The application process is a three-step process:

Step 1 – Renewal application form submission with signature applied without the fee amount

Step 2- Continued education hours summary form and certificates submission

Step 3 – Payment information is populated and communicated, final signature applied with payment


Before You Start
If your practice licence has expired over five (5) years or more, you may be required to submit a new application. Please contact the Registrar for further information/ instructions at hpc@gov.ky.

Please note that you are required to provide the documents that are specific to your own council.

In addition, you will need to:

1. Provide support for any changes to affiliate/employer which includes a new letter of affiliation or proof of name changes marriage certificate, divorce decree, etc.

2. Provide written evidence of any convictions, disciplinary actions, investigations or proceedings which are pending or completed.

3. Have a copy of your current practice licence for reference purposes.

Required Documents

  • Continued Medical Education Certificates (CME) or Continued Education certificates (CE).
  • Copy of current Malpractice Insurance.
  • Written evidence of any convictions, disciplinary actions, investigations or proceedings which are pending or completed.
  • A copy of your current practice licence for reference purposes.

Please note: Completing the required online summary forms is a mandatory part of your Practitioner Renewal application. Ensure you finish and submit the form in its entirety before proceeding with final submission.
You can find all Practitioner Renewal summary forms under the Resources menu in the top navigation. Simply hover over Resources and click Forms to access and complete the required online forms.
